Fayette CARE Clinic works closely with other medical facilities when patients require expertise or equipment not available on site. Counseling provided by physicians and other medical professionals offers information with a goal of preventative care. Counseling is based on the individual needs of each patient. Seminars and educational literature on specific medical topics - including the impact of lifestyle on personal health - is also offered. Registered dietary and nutrition experts provide educational opportunities in the form of literature, personal counseling, and classroom sessions for those with special dietary needs.
In 2008, Kathy Leoffler started our diabetic program which includes group visits, individual patient visits, peer coaching, and education. In 2010 alone, Kathy provided education about diabetic health care for 88 of our patients and tracked their A1C levels. Of those patients, only 5 visited the ER that year for diabetes-related issues. In October 2011, Fayette CARE Clinic received a generous grant from the American Medical Association Foundation to further enhance our diabetic programs.
